Key Responsibilities

The Physical Therapist will perform home visits to adult and geriatric clients to conduct diagnostic tests, implement individualized care plans, and provide rehabilitative interventions. They are responsible for educating clients and families while accurately documenting care using electronic medical record systems.

Requirements

Candidates must hold a current Virginia Physical Therapist license and have graduated from an accredited physical therapy program. Strong organizational, interpersonal, and computer skills are required to manage independent home health visits effectively.
