Key Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities to improve mobility and independence. Perform evaluations, develop individualized care plans, and utilize telehealth services to extend patient access.

Requirements

Must hold a valid Physical Therapy license in the state of practice and a degree from an accredited Physical Therapy program. Current CPR certification and basic computer/EMR proficiency are required.
