Occupational Therapist (OTR/L) - CAPABLE Program & Adult Day Health Care (ADHC)

Location: Community-Based/Home Visits

Department: Adult Day Health Care (ADHC) / CAPABLE Program

Areas of Assignment: Manhattan and Bronx

Reports To: Project Coordinator

About the Role

We are seeking a compassionate and experienced Occupational Therapist (OTR/L) to join our innovative CAPABLE (Community Aging in Place, Advancing Better Living for Elders) Program within our Adult Day Health Care (ADHC) services. An incentive bonus of $ 3000 will be given to the full time hire, and 50% for part time, to be distributed as follows: 50% after 6 months of service and 50% after 1 year of service.

This exciting community-based role focuses on helping older adults maintain independence, improve safety, and age in place successfully. Working collaboratively with a Registered Nurse and the interdisciplinary care team, the Occupational Therapist will conduct in-home assessments, identify barriers to daily functioning, and recommend interventions and home modifications designed to reduce hospitalization, prevent long-term care placement, and enhance overall quality of life.

The ideal candidate is passionate about serving older adults, skilled in home-based assessments, and committed to empowering participants to achieve their personal health and wellness goals.

Key Responsibilities

  • Conduct comprehensive screenings and assessments for individuals referred to the CAPABLE Program.
  • Perform in-home evaluations to assess participants' functional abilities, home safety, and environmental challenges.
  • Evaluate physical, cognitive, emotional, and sensory functioning.
  • Assess participants' ability to perform Activities of Daily Living (ADLs) and develop individualized intervention plans.
  • Identify safety risks and recommend home modifications to improve accessibility, independence, and quality of life.
  • Prepare work orders and collaborate with vendors and team members regarding approved home modifications.
  • Educate and train participants, family members, and caregivers on adaptive equipment, therapies, and safety strategies.
  • Complete all required assessments, progress notes, and documentation in compliance with CAPABLE program requirements.
  • Maintain accurate, timely, and confidential clinical records.
  • Collaborate with the interdisciplinary CAPABLE team to support participant goals and quality improvement initiatives.
  • Coordinate and manage scheduling for home assessments and participant visits.
  • Prepare and maintain program statistics and required reports.
  • Provide rehabilitation therapy services within the ADHC program as assigned.
  • Participate in professional development opportunities, trainings, and continuing education programs.
  • Maintain regular communication with the Project Coordinator regarding participant progress and program needs.

Training

The selected candidate will participate in a 10-hour virtual CAPABLE training program focused on assessment, care planning, implementation, and evaluation of CAPABLE services.

Qualifications

Required

  • Master's Degree in Occupational Therapy from an accredited program.
  • Current and unrestricted New York State Occupational Therapy License.
  • Minimum of 3 years of home care experience in a Certified Home Health Agency (CHHA), Long-Term Home Health Care Program (LTHHC), or similar community-based setting.
  • Strong knowledge of occupational therapy principles, treatment techniques, adaptive equipment, and safety practices.
  • Experience conducting home safety assessments and recommending home modifications.
  • Proficiency with electronic health records and standard computer applications.
  • Valid driver's license and willingness to travel to participant homes.
  • Strong organizational, documentation, and time-management skills.
  • Excellent communication and interpersonal skills.

Preferred

  • Bilingual skills.
  • Experience working with older adults in community-based or geriatric care settings.
  • Knowledge of aging-in-place programs and evidence-based interventions designed to reduce hospitalizations and improve independence.
