• Answering service takes after hours call first, then dispatched out to team

• NextGen EMR

• 12 patients per day on average for follow

• 6 patients per day on average for new patients Coverage Areas:

Requirements

• Hospitalist or home health experience

• Experience and training in quality improvement and patient safety

• Able to create personal relationships and build human connections

• Communicate effectively with patients, family, medical power of attorney, primary care provider and all individuals involved in the patient’s care
