Key Responsibilities
- Answering service takes after hours call first, then dispatched out to team
- NextGen EMR
- 12 patients per day on average for follow
- 6 patients per day on average for new patients Coverage Areas:
Requirements
- Hospitalist or home health experience
- Experience and training in quality improvement and patient safety
- Able to create personal relationships and build human connections
- Communicate effectively with patients, family, medical power of attorney, primary care provider and all individuals involved in the patient’s care
