Job Description

Practice Owner — Physical Therapist or Occupational Therapist

Northern California | Independent Living, Assisted Living & Memory Care

Prava Therapy

  • What if you could own the clinic—and have Prava in the room with you?
  • What if you could walk into a senior living community and say:

“This is my practice.”

  • Your clinical vision.
  • Your relationships.
  • Your reputation.
  • Your community.
  • Your team.
  • Your growth.
  • And behind you?
  • Prava.
  • Not just as your technology provider.
  • Not just as your billing partner.
  • As your operating partner.
  • We're looking for an entrepreneurial Physical Therapist or Occupational Therapist to become a Prava Practice Owner serving a premier senior living community portfolio in Northern California, including Independent Living, Assisted Living, and Memory Care.
  • This isn't a traditional therapy job.
  • And it isn't traditional clinic ownership.
  • It's a partnership.
  • You own and lead your practice.
  • Prava brings the infrastructure, expertise, and operational support to help you build it.
  • And together, you and Prava become the therapy partner to the community operator.

What If You Owned the Clinic?

  • Imagine having a practice where:
  • You know the residents.
  • You know the families.
  • You know the community leadership.
  • You build the clinical culture.
  • You develop your team.
  • You create the clinical vision.
  • You build relationships with the operator.
  • You and Prava work together to solve problems and create opportunities.
  • You have a direct connection between exceptional care, community partnership, and practice growth.
  • You aren't just filling a caseload.
  • You're building a practice—and building a partnership with the community you serve.

You Own the Practice.

  • PRAVA PARTNERS WITH YOU TO OPERATE IT.
  • This is what makes the Prava model different.
  • You don't have to figure out everything alone.

Prava provides the operating infrastructure and expertise that allow you to focus on being the kind of Practice Owner a community actually wants to partner with.

Prava Supports

  • Technology and EHR
  • AI-enabled documentation
  • Credentialing and contracting
  • Billing and revenue cycle
  • Marketing
  • Clinical standards
  • Operational systems
  • Reporting and business intelligence
  • Leadership and ongoing coaching
  • But the relationship with the community?
  • That's something we build together.
  • You bring the clinical credibility and community presence.
  • Prava brings the infrastructure and operating expertise.

Together, we work with the community operator to understand their priorities, identify opportunities, solve problems, and build a therapy program that delivers real value to residents and the organization.

You own the practice. We operate in partnership with you.

You and Prava Become the Operator's Therapy Partner.

This is bigger than delivering therapy.

A great Practice Owner understands that the community operator has goals, challenges, priorities, and a vision for the resident experience.

You'll work alongside Prava to understand those priorities and translate them into an exceptional therapy program.

Together, We Partner with the Operator Around

  • Resident outcomes and experience
  • Clinical quality
  • Community integration
  • Referral pathways
  • Staff education
  • Functional wellness
  • Communication and collaboration
  • Program performance
  • Growth opportunities

The Evolving Needs of the Resident Population

  • You're not an outside vendor walking in to provide visits.
  • You're part of the community's clinical ecosystem.

Your Community Becomes Your Clinic.

  • This is not an outpatient clinic where residents drive in, receive treatment, and disappear.
  • Your practice lives inside the community.
  • You'll know the residents.
  • You'll understand the environment.
  • You'll see how people function throughout their day.

You'll build relationships with leadership, nursing, wellness, life enrichment, caregivers, residents, and families.

And you'll have the opportunity to create a therapy program that becomes part of the community—not a service that simply operates inside it.

That's concierge-style senior living therapy.

What You'll Own

As a Prava Practice Owner, you'll be responsible for the clinical vision, community relationship, and day-to-day leadership of your practice—with Prava alongside you as an operating partner.

You'll

  • Establish yourself as the trusted clinical leader within the community.
  • Maintain a strong clinical presence and active caseload.
  • Build meaningful relationships with residents, families, community leadership, and care teams.
  • Develop a strong partnership with the community operator alongside Prava leadership.
  • Understand the operator's priorities and translate them into clinical and operational opportunities.
  • Identify opportunities for therapy and early intervention.
  • Develop a culture of exceptional clinical care.
  • Mentor and develop your therapy team.
  • Lead clinical communication and community partnerships.
  • Participate in community meetings and interdisciplinary collaboration.
  • Help educate community staff about therapy, functional decline, mobility, cognition, safety, and appropriate referral opportunities.
  • Monitor clinical quality, resident outcomes, documentation, and program performance.
  • Build referral relationships and strengthen the visibility of your practice.
  • Identify opportunities to expand services and improve the resident experience.
  • Work with Prava leadership to establish goals, review performance, and execute growth strategies.
  • Collaborate with Prava on operational, financial, clinical, and growth priorities.
  • Represent your practice and Prava with professionalism, integrity, and clinical credibility.

This Is for the Therapist Who Has Always Thought...

“I could do this better.”

  • You've seen what works.
  • You've seen what doesn't.
  • You've watched great clinicians get buried under bad systems.
  • You've watched talented therapists get pulled away from clinical leadership because they're expected to be clinicians, recruiters, billers, marketers, schedulers, and administrators all at once.
  • You've thought about opening your own practice.
  • Maybe you've even started looking into it.

And Then You Realized

  • There's a lot that comes with owning a practice.
  • That's where Prava comes in.

Own the Clinic.

DON'T BUILD THE INFRASTRUCTURE. BUILD THE PARTNERSHIP.

Prava gives you the platform, people, systems, and support to operate a modern therapy practice without starting from zero.

You Bring

  • The clinical expertise.
  • The leadership.
  • The relationships.
  • The entrepreneurial mindset.

We Bring

  • The operating system.
  • The technology.
  • The infrastructure.
  • The operational expertise.
  • The support.

And Together

We build the partnership.

You're Not Just a Therapist.

  • You're a builder.
  • You're a connector.
  • You're a clinician.
  • You're a leader.

You're Someone Who Notices an Opportunity and Thinks

“What could this become?”

  • You don't need to have owned a practice before.
  • You do need to think like an owner.

That means you're comfortable taking initiative, building relationships, solving problems, developing people, and being accountable for the success of your community.

You Also Understand Something Important

  • Great senior living partnerships aren't transactional.
  • They're built on trust.
  • Consistency.
  • Communication.
  • Clinical credibility.
  • And showing up.

The Residents Come First.

  • Ownership doesn't change what matters most.
  • The resident does.

You Believe

  • Every intervention should have a functional purpose.
  • Residents should be appropriately challenged—not underestimated.
  • The environment should be part of the clinical solution.
  • Caregiver education is part of treatment.
  • Outcomes matter more than minutes.
  • Dignity and autonomy are clinical priorities.
  • You want to build a practice where those principles aren't just written on a wall.
  • They're how the practice operates.

What Success Looks Like

  • Residents know you.
  • Families trust you.
  • Your clinicians want to work with you.
  • Community leadership sees you and Prava as partners.
  • The operator knows they can call you.
  • Your practice is growing.
  • Your team is developing.
  • Your clinical outcomes are strong.
  • And therapy becomes an integral part of the community's overall approach to helping residents live well.
  • That's ownership.
  • Not just owning a business.
  • Owning the impact you make—and building it with a partner who has your back.

So... What If?

  • What if you could own the clinic?
  • What if you could build the team?
  • What if you could create the culture?
  • What if you could develop the community relationship?
  • What if you could grow the practice?
  • What if you could have the entrepreneurial upside without spending years building the infrastructure?
  • What if you didn't have to do it alone?

Your practice. Your community. Your clinical vision.

Prava's infrastructure. Prava's expertise. Prava by your side.

  • Together, we build something exceptional.
  • If that makes you a little excited—and maybe a little nervous— good.
  • We should talk.
