Physical Therapist - Outpatient

We are currently searching for a Physical Therapist to work in our Health Center. In this role, the Physical Therapist will provide patient/resident assessment, care planning, direct treatment and consultation to a general rehab caseload. The Physical Therapist will participate in the clinical team process and provide teaching and training to care giver staff. There will also be involvement with the clinical program development.

This Position Is Responsible For

• Evaluation and treatment of physical conditions in order to restore functions and improve mobility as indicated and ordered by MD. Design and execution of Care Plan.
• Conducting assessment and screens to determine the need for therapy.

Providing treatment modalities including electrical stimulation, hot/cold packs and therapeutic massage.

• Maintain timely and accurate documentation, including updating information in the systems.
• Performing Resident Home Assessments as part of the discharge process.
• Performing Resident's screening during the admission process as well as quarterly and annual screenings to determine the need for therapy.

Qualifications

Education

Current State of Florida Physical Therapist license

Experience and Basic Knowledge

One year of long-term care desirable. Med-surg, Home Health experience helpful.

Working with handicapped or developmental disabled, or older adults preferred.
