BrightStar Care is looking for a Part-Time Physical Therapist Assistant (PTA) to support our expanding home health team in the Burnsville area and nearby communities. This position is a great match for someone who enjoys flexible scheduling, independent work, and providing meaningful one-on-one care in clients’ homes.

Because this role involves traveling between visits, applicants should feel comfortable driving throughout the service area. Hours and visit volume may vary based on referrals and territory needs, making this an excellent opportunity for someone who appreciates a dynamic, adaptable workload.

Key Responsibilities

• Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
• All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within patient’s capabilities.
• Participates with all other home care personnel in patient care planning.
• Takes initial history and makes initial evaluation.

Consults with Physicians Regarding Patient’s Progress

• Participates in quality assessment and performance improvement activities as assigned.
• Prepares clinical and progress notes.
• Helps develop the plan of care and revise, as necessary.
• Consults with family and agency personnel.
• Ability to do extensive bending, lifting and standing on a regular basis.

Requirements

A graduate of a PTA Program recognized by the American Physical Therapy Association or other National accepted Council or Association

• An active license to practice Physical Therapy in MN
• A minimum of four (4) years’ experience as a Physical Therapist Assistant, home care experience a plus
• Must have criminal background check
• Must have current CPR certification
• Must have active driver's license and clear driving record
• Computer skills.
