What You'll Do

  • Evaluate patients' functional abilities, mobility, safety awareness, and home environments.
  • Develop individualized treatment plans focused on maximizing independence and quality of life.
  • Provide therapeutic interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Assess and recommend adaptive equipment, home modifications, and safety strategies.
  • Train patients, caregivers, and families on techniques that support successful home management.
  • Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals.
  • Complete accurate and timely clinical documentation in accordance with agency standards.

Qualifications

  • Graduate of an accredited Occupational Therapy program.
  • Current Oregon Occupational Therapist license.
  • Current CPR certification or ability to obtain.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Home Health experience preferred but not required.
  • Strong clinical judgment, organizational skills, and communication abilities.
