Case Manager - Pediatric Behavioral Health - Columbia – Grant Funded

Key Responsibilities

The Case Manager will provide systems care coordination, targeted referrals, and supportive engagement for pediatric patients and their families. They will work within a multi-disciplinary team to address emergent mental health needs and participate in community engagement activities.

Requirements

Candidates must hold a Bachelor of Arts or Sciences degree in Social Work, Psychology, or an equivalent field. Experience working with child abuse and intimate partner violence is required, and bilingual Spanish speaking skills are highly preferred.
