Job Details: Job Location: East Valley Medical Respite Center - Mesa, AZ, Salary Range: Undisclosed, Summary of Position: Each patient that is referred to the Physical Therapy Program by a CTC physician will receive a complete evaluation by a registered Physical Therapist. Each patient will then receive treatment according to an individualized treatment plan. All services are to be recorded in the Electronic Health Record (EHR). Essential Duties: Clinical

Responsibilities

Evaluate and treat each physician referred patient. Design and document a treatment plan based on the results of the evaluation. Document each visit in the Electronic Health Record (EHR). Document progress made toward treatment goals.

Complete a discharge summary when goals are attained or discharged according to the wishes of the patient. Attend weekly multi-disciplinary rounds; participate in the discussion surrounding patients plan of care, goals and continued therapy.

Other responsibilities/accountabilities: Duties and responsibilities may be added, deleted or changed to meet program needs, at the discretion of the Medical Director.

Qualifications

Qualifications: Basic Knowledge/Skills: Understanding of substance and Mental Health issues Interpersonal and communication skills sufficient to effectively interact with patients, physicians, and health team members required Computer proficiency inclusive of ability to access, enter, and interpret computerized data/information required Basic Life Support certification required (BLS) Experience and Education: License to practice physical therapy in Arizona required Masters of Physical Therapy required. Key Responsibilities

The Physical Therapist will evaluate and treat patients referred by physicians, designing and documenting individualized treatment plans in the Electronic Health Record. They are also responsible for tracking patient progress, attending multi-disciplinary rounds, and completing discharge summaries.

Requirements

Candidates must hold a valid license to practice physical therapy in Arizona and possess a Master's degree in Physical Therapy. Additionally, a Basic Life Support (BLS) certification and proficiency in computer systems for data entry are required.
