Inpatient Social Worker - Per Diem

Responsibilities

Position Summary

The Social Worker supports patients in meeting their psychosocial needs and achieving emotional health. They facilitate treatment planning, conduct assessments, provide psycho-education, offer short-term therapy, lead group sessions, and coordinate discharge planning with community resources. Using evidence-based practices, they help clients reach their optimal mental and emotional well-being.

Required

• Licenced Agency Affiliated Counselor (AAC), or Social Worker Associate Independent Clinical (LSWAIC)
• BLS, First Aid, Handle with Care, Verbal De-escalation (INBH Provided)

Preferred: Licensed Mental Health Counselor (LMHC), Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), Licensed Advanced Social Worker (LASW), Licensed Psychologist

Qualifications

Essential Duties & Responsibilities

Conduct psychosocial assessments to evaluate patients' emotional, behavioral, developmental, social, and support needs.

Provide individual, family, and group interventions to support treatment goals and patient progress.

Develop, implement, and update individualized treatment plans in collaboration with patients, families, and the interdisciplinary treatment team.

Facilitate treatment planning meetings and coordinate care with physicians, nursing staff, and other clinical disciplines.

Assist patients and families with identifying resources and addressing psychosocial, educational, financial, and discharge planning needs.

Monitor patient progress, evaluate treatment effectiveness, and adjust interventions as appropriate.

Facilitate family meetings and support ongoing communication between patients, families, and the treatment team.

Maintain timely, accurate, and compliant clinical documentation, including assessments, treatment plans, progress notes, and discharge planning activities.

Ensure confidentiality and compliance with all applicable regulatory, accreditation, and organizational standards.
