Emplify Health by Gundersen is adding a fulltime Outpatient Occupational Therapist to our Onalaska therapy team to primarily work with and treat our Neuro & Lymphedema patient populations in our Complex Care Clinic as we bring patient-centered care closer to home. This position may also include seeing patients within the Therapy Department in addition to the Complex Care Clinic. Upcoming OT graduates are encouraged to apply.

What You'll Do

As an Occupational Therapist (OT) on our team, you will provide comprehensive, patient-centered occupational therapy services for patients enrolled in Complex Care Services (CCS).

Working as a member of an interdisciplinary care team, the OT evaluates and addresses functional, physical, cognitive, behavioral, environmental, and psychosocial factors that impact a patient’s ability to safely and effectively participate in daily activities and manage their health.

The OT partners with patients to identify meaningful goals and develop individualized strategies that promote independence, safety, self-management, and quality of life.

Services may include functional assessment, intervention, education, adaptive strategies and equipment recommendations, energy conservation, activity modification, home and community safety, caregiver education, and coordination with other members of the care team.

Complex Care Services (CCS) is an interdisciplinary care model designed to support patients with multiple chronic conditions, frequent healthcare utilization, and complex care needs.

Through proactive, relationship-based care, the CCS team partners with patients to improve health outcomes, enhance the patient experience, and reduce avoidable emergency department visits and hospitalizations.

Within CCS, the Occupational Therapist contributes to the patient’s care plan by helping the team understand how health conditions and other barriers affect the patient’s ability to function in daily life.

The OT collaborates closely with the clinician, RN Care Coordinator, Medical Assistant, Social Worker, Behavioral Health Consultant, Registered Dietitian/Diabetes Care Specialist, Pharmacist, and Physical Therapist, as well as other community or specialty resources to support coordinated, patient-centered care.

  • Fulltime, 80 hours biweekly (1.0 FTE)
  • Monday - Friday, 8a-4:30p
  • 60-minute appointment lengths
  • Starting salary of $74,214 and up, commensurate with your years of work experience

What You'll Need

  • Bachelor's degree, or higher, in Occupational Therapy
  • Occupational Therapist - Wisconsin (OT) license
  • Current Infant-Adult BLS for Health Care Providers from American Heart Association or American Red Cross
  • Lymphedema Certification desired
  • Neuro experience preferred
