Physical Therapist- Home Health

Key Responsibilities

The Physical Therapist will perform home health visits to provide rehabilitative care, diagnostic testing, and individualized care plans for adult and geriatric clients. They are responsible for educating clients and families while accurately documenting care using a tablet.

Requirements

Candidates must hold a current Virginia Physical Therapist license and have graduated from an APTA or AMA-approved program. Strong interpersonal, organizational, and computer skills are required to manage independent home visits effectively.
