Hospice Medical Social Worker

Our team is devoted to walking alongside patients and families during one of life’s most meaningful journeys — and we’re looking for a Hospice Social Worker (MSW) who shares that calling.

As a Hospice Social Worker, you won’t just be “filling a role” — you’ll be a lifeline of support, helping patients and their loved ones navigate emotional, social, and practical challenges with warmth and understanding. You’ll bring comfort in moments of uncertainty, empower families with resources, and provide a listening ear when it matters most.

What You’ll Do as a Social Worker

• Provide emotional support, counseling, and education to patients and families
• Guide families through difficult decisions and connect them with community resources
• Advocate for patient needs as part of an interdisciplinary care team
• Support patients’ wishes, dignity, and spiritual well-being
• Help ensure that no patient or family ever feels alone in their hospice journey
• Who you are as a Social Worker Social Worker:
• Master’s Degree in Social Work (MSW)
• A heart for service, empathy, and compassion
• Experience in hospice, healthcare, or counseling preferred
• Strong communication and listening skills
• A collaborative, team-focused spirit
