Children's Health Licensed Clinical Social Worker

Key Responsibilities

• Receives referrals from clinicians in the Children’s Health clinics to assess patients’ developmental, behavioral and mental health needs. Work as part of an integrated medical home to include pediatric medical providers, dieticians, social workers and referral coordinators.

• Develops and implements treatment plans and prioritizes goals, works with parents and children ages birth to 21.

• Provides ongoing counseling and psycho-educational support to patients and parents. Provides crisis services by managing mental health emergencies when they arise. Manages ADHD screening program, works closely with referral coordinator and school personnel to complete process.

• Attend meetings for foster care program, medical provider meetings and staff meetings. Collaborate with outside agencies to best serve patient population.

• Emergency Service Worker Statement In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected.

• Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency.

Benefits & Compensation

• Salary: $68K - $115K
