Carezzi is seeking a dedicated Physical Therapist (PT) to provide skilled, compassionate therapy services to clients in their homes while enjoying the flexibility of a 1099 independent contractor opportunity.

Responsibilities

• Evaluate patient functional status and develop individualized treatment plans
• Provide skilled physical therapy services in the home setting
• Implement therapeutic exercises and mobility programs
• Assess home safety and recommend modifications to reduce fall risks
• Educate patients and caregivers
• Monitor patient progress and adjust treatment interventions
• Communicate with physicians and interdisciplinary care team members
• Document services provided according to Carezzi requirements
• Maintain professionalism, compliance, and client confidentiality

Qualifications

• Current Physical Therapist (PT) license in the state of practice
• Graduate of an accredited Physical Therapy program
• Valid driver license and reliable transportation
• Current CPR certification preferred
• Strong communication and documentation skills
• Ability to work independently in a home-based care setting
• Pass background screening requirements
