Key Responsibilities

• Patient Evaluation & Care Planning.

• Perform initial and ongoing patient evaluations to assess mobility, strength, balance, endurance, and functional limitations.

• Complete comprehensive assessments and participate in OASIS-related coordination as applicable.

• Develop, implement, and update individualized physical therapy treatment plans.

Requirements

• Graduate of an accredited Physical Therapy program required. Current state Physical Therapist (PT) license in good standing required. Minimum of one (1) year of experience in physical therapy required; home health, acute care, or rehabilitation experience preferred.

• Current CPR certification required.

• Valid driver’s license, reliable transportation, and auto insurance.

• Ability to pass background screening and all compliance requirements.

Work Schedule

• Per Diem position.
