Key Responsibilities
- The role will assess, plan, and implement support plans to meet the overall needs of the entire family.
- The role empowers patients/families to utilize resources to ensure necessary medical care and to achieve optimal psychosocial functioning.
- This role will also assist with management of the Family Leadership Committee as well management and educational support of Trauma Informed Care within the Complex Care Program.
