Key Responsibilities

Include: Works closely with primary care providers to assess patient needs and develop appropriate support and/or interventions.

Behavioral services include but are not limited to; individual therapy, behavioral health education, support groups, information on other community resources and linkages to other resources.

Provides case management support as necessary to ensure that client’s medical, mental health and social service needs are being addressed and met.

Provides behavioral health consultation to primary care providers and support staff.

Requirements
