Key Responsibilities
- Include: Works closely with primary care providers to assess patient needs and develop appropriate support and/or interventions.
- Behavioral services include but are not limited to; individual therapy, behavioral health education, support groups, information on other community resources and linkages to other resources.
- Provides case management support as necessary to ensure that client’s medical, mental health and social service needs are being addressed and met.
- Provides behavioral health consultation to primary care providers and support staff.
