Key Responsibilities

• Include: Works closely with primary care providers to assess patient needs and develop appropriate support and/or interventions.

• Behavioral services include but are not limited to; individual therapy, behavioral health education, support groups, information on other community resources and linkages to other resources.

• Provides case management support as necessary to ensure that client’s medical, mental health and social service needs are being addressed and met.

• Provides behavioral health consultation to primary care providers and support staff.

Requirements

• LMFT with a valid, unrestricted California license.

• Current CPR/BLS certification.

Benefits & Compensation

• Pay rate: $55 - $70/hour.
