Key Responsibilities
- Interviews potential residents to complete a social history and cognitive assessment in determining level of care.
- Provides follow-up Social Work visits to newly admitted residents and documents such visits within ninety (90) days after arrival in the resident’s medical record.
- Offers direct social work interventions to residents and their families and provides appropriate measures for helping residents in their personal adjustment, emotional and social well-being and accessing of appropriate facility and community referrals and resources.
- Documents in the clinical chart appropriate conversations, meetings, and interventions with residents, families/POA, and staff concerning residents as required by Federal and State regulations and TVA policy.
Requirements
- A BSW or MSW degree from an accredited university; prefer a licensed clinical social worker (LCSW).
- Must possess computer skills; prefer experience with electronic medical records.
- Prefer to have at least three (3) years experience in social work with at least one (1) year in work with the geriatric population.
