Key Responsibilities

• Interviews potential residents to complete a social history and cognitive assessment in determining level of care.

• Provides follow

• up Social Work visits to newly admitted residents and documents such visits within ninety (90) days after arrival in the resident’s medical record.

• Offers direct social work interventions to residents and their families and provides appropriate measures for helping residents in their personal adjustment, emotional and social well

Requirements

• Counsels residents and families to enable them to discuss and understand their feelings, attitudes, fears and anxieties related to problem solving and to explore alternative solutions.

• Coordinates clinical social work services with the hospice social worker for residents who are hospice patients.

• Develops and maintains contact with appropriate community agencies and services to promote cooperation and facilitate related referrals.
