About the Role

Join Pinnacle Home Care as a Physical Therapist – Home Health, Flexible Schedule in United States.

Key Responsibilities

Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.

Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.

Collaborate with the interdisciplinary care team to optimize patient outcomes.

Provide education and support to patients and families/caregivers on therapy programs.

Requirements

Must have a current PT license (state

Graduate of an accredited physical therapy program.

Valid BLS certification.

Minimum one year of PT experience (home health experience preferred).
