Key Responsibilities

The Nurse Practitioner will conduct comprehensive patient assessments and develop individualized treatment plans within post-acute and long-term care settings. They are responsible for managing medical conditions, prescribing medications, and coordinating care with interdisciplinary teams to ensure patient safety and continuity.

Requirements

Candidates must hold a Master’s degree or Doctor of Nursing Practice (DNP) and possess a current, unrestricted Virginia Nurse Practitioner license. Experience in geriatric, post-acute, or primary care settings is preferred, along with strong diagnostic and clinical assessment skills.
