Key Responsibilities

• Conduct biopsychosocial assessments for new clients, coordinate and monitor services, including home care, m.

• Provide support, guidance, and oversight to assigned Case Managers to ensure effective case management servi.

• Conduct periodic case reviews with Case Managers to monitor service coordination, documentation quality, and.

Requirements

• MSW degree and experience working with older adults.

• Knowledge of resource and benefit programs.

• Excellent organizational, communication, outreach and advocacy skills and the ability to work as a team.

• Computer literacy including proficiency in Microsoft Office applications.

Benefits & Compensation

• Salary: $65K

• 37 days PTO.
