Hillcrest Health & Living is looking for a compassionate, motivated, and patient-focused Physical Therapist to join our Hillcrest Rehab Services team on a part-time basis.

Our rehabilitation team helps patients regain strength, improve mobility, reduce pain, restore function, and work toward greater independence. As a Physical Therapist, you will use your clinical expertise to evaluate individual needs, develop evidence-based treatment plans, and collaborate with an interdisciplinary team to help each patient achieve meaningful goals and successful transitions of care.

What You’ll Do

As a Physical Therapist, you will evaluate and treat patients with a variety of functional and mobility needs while developing individualized plans of care focused on meaningful outcomes.

You will

• Complete comprehensive physical therapy evaluations using appropriate tests and measures.
• Develop individualized plans of care based on each patient’s needs, abilities, goals, and expected outcomes.

Provide evidence-based physical therapy interventions to improve mobility, strength, balance, function, and independence.

Progress treatment plans based on patient response and established goals.

Educate patients and family members regarding treatment plans, exercises, mobility, safety, and ongoing care needs.

Adapt treatment and education based on individual cognitive abilities, learning needs, and cultural considerations.

• Complete accurate and timely documentation of therapy services.
• Collaborate with physicians, nurses, therapy team members, and other disciplines to coordinate patient care.

Communicate patient progress, changes in condition, concerns, and recommendations with the interdisciplinary team.

Develop effective transition and discharge plans, including recommendations for continued services and equipment.

Provide appropriate handoff communication when patients transition between care settings.

Supervise, collaborate with, and delegate appropriate care to Physical Therapist Assistants, therapy techs, and other support personnel.

Identify patients who may benefit from additional medical or therapy services and complete appropriate screenings.

Manage an effective patient schedule while considering patient preferences, clinical needs, and treatment goals.

• Identify potential patient or facility safety risks and take appropriate action.
• Participate in professional development, clinical education, and other professional activities as appropriate.

• Education & Credentials
• Graduate of an accredited Physical Therapy program required.
• Current Physical Therapist license in Nebraska or an applicable compact state required.
• American Heart Association Basic Life Support (BLS) certification required.

At least two years of Physical Therapy experience in long-term care, post-acute care, hospital, home care, or a similar setting preferred.

ABPTS board certification in geriatric, orthopedic, or neurological physical therapy preferred.

• Physical & Other Requirements
• Must be able to speak, read, and write English.
• Must be able to independently lift up to 35 pounds.

Must be able to perform repetitive sitting, standing, bending, reaching, lifting, walking, and other movements associated with providing therapy services.

• Must be flexible and calm when supporting patients and families during stressful situations.
• Must have strong attention to detail and the ability to work effectively in a fast-paced clinical environment.
• Must be able to utilize technology and available resources efficiently and effectively.
• Must follow established infection-control, patient-handling, documentation, and safety procedures.
• Possible exposure to communicable diseases and bodily fluids.
• Other duties as assigned.
