Case Manager, RN (FH) (Per-Diem Weekends)

Key Responsibilities

The Case Manager facilitates patient care from pre-admission through post-discharge by coordinating with the interdisciplinary team to ensure medically appropriate and cost-effective treatment. They are responsible for concurrent utilization management, discharge planning, and ensuring compliance with regulatory standards.

Requirements

Candidates must hold a Bachelor's degree in Nursing and a current New York State Registered Professional Nurse license. A minimum of five years of clinical experience as a registered nurse is required, with preference given to those with utilization management or discharge planning experience.
