Job Description
Elite Home Health & Hospice is seeking a compassionate and dedicated Medical Social Worker (BSW or MSW) to join our interdisciplinary team serving patients across both our Home Health and Hospice service lines. This role provides an opportunity to make a meaningful impact by helping patients and families navigate the social, emotional, financial, and community resource challenges that often accompany illness and healthcare transitions.

Position Summary
The Medical Social Worker assists patients and families in addressing social, emotional, psychosocial, financial, and environmental concerns related to illness, disability, end-of-life care, and recovery. Working as part of an interdisciplinary team, the Social Worker helps identify barriers to care, connects patients with community resources, and provides counseling and support throughout their healthcare journey.
Essential Responsibilities
Home Health
- Complete psychosocial assessments and identify patient and family needs.
- Assist patients in overcoming barriers to achieving healthcare goals.
- Connect patients and families with community resources and support services.
- Provide counseling related to adjustment, chronic illness, disability, financial concerns, and caregiver stress.
- Collaborate with physicians, nurses, therapists, and community partners to coordinate patient care.
- Assist with advance directives and healthcare planning when appropriate.
- Educate patients and families about available services and community programs.
- Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements.
Hospice
- Provide emotional support and counseling to patients and families facing terminal illness.
- Assist families with end-of-life planning and resource coordination.
- Support patients and caregivers through the grieving process and anticipatory grief.
- Participate in hospice interdisciplinary group meetings and care planning.
- Provide advocacy and crisis intervention when needed.
- Assist with transitions of care and discharge planning.
- Support patients' rights, goals, and preferences throughout the hospice journey.
Qualifications
- Bachelor's degree in social work (BSW) required; MSW welcomed and encouraged to apply.
- Current Idaho social work licensure, or ability to obtain required licensure, preferred.
- Experience in healthcare, home health, hospice, hospital, case management, or related social work settings preferred.
- Knowledge of community resources, healthcare systems, and psychosocial assessment practices.
- Strong communication, organization, and interpersonal skills.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Valid driver's license, reliable transportation, and proof of auto insurance.
