Key Responsibilities
The Nurse Practitioner will consult with patients and families regarding disease processes and end-of-life care while collaborating with the interdisciplinary team to develop and implement care plans. They are responsible for monitoring patient needs, communicating changes to physicians, and ensuring all clinical documentation is submitted in a timely manner.
Requirements
Candidates must hold a current state license as a Registered Nurse and Advanced Practice Nurse, along with a Baccalaureate degree in Nursing. A minimum of two years of clinical experience as an APN is required, with preference given to those with hospice or home health backgrounds.
