Key Responsibilities

The Physical Therapist will plan and administer personalized rehabilitation programs for patients in their homes to improve strength, mobility, and independence. They are responsible for documenting patient progress, coordinating with the care team, and overseeing the performance of physical therapy assistants and home health aides.

Requirements

Candidates must hold a current and unrestricted Physical Therapy license and a valid CPR certification. Experience collaborating with a team of healthcare professionals is required, and one year of experience as a Physical Therapist is preferred.
