Prava Therapy is seeking a full-time Occupational Therapist to join our team within a vibrant Independent Living community in Apex, North Carolina.

This is an opportunity to practice meaningful, relationship-driven therapy in a community where residents value independence, wellness, connection, and an active lifestyle.

Your Role

As an Occupational Therapist, you'll provide skilled clinical care while becoming a trusted partner within the community.

You'll

  • Perform comprehensive evaluations and develop individualized plans of care.
  • Provide skilled treatment addressing ADLs, IADLs, functional mobility, upper extremity function, adaptive strategies, cognitive-functional performance, and daily living independence.
  • Connect therapy interventions to meaningful, resident-centered goals.
  • Build strong relationships with residents and understand what matters most to them.
  • Identify changes in function and opportunities for early intervention.
  • Educate residents, families, caregivers, and community team members on home/apartment safety, adaptive equipment, energy conservation, and independence.
  • Collaborate with physicians, community leadership, wellness professionals, and other members of the care team.
  • Monitor progress and modify treatment plans based on clinical presentation and functional outcomes.
  • Maintain timely, accurate, and compliant documentation.
  • Participate in care coordination and discharge planning as appropriate.
  • Contribute to a culture of clinical excellence, compassion, professionalism, and continuous learning.
