• will also be working with our SPIRE project for enhanced AWVs and assisting to identify patients with social and behavioral health needs.

• This social worker does short term therapy and if the patient needs a higher level of care they help to coordinate the services to either Griffin IOP or outside services.

• This social worker also works in conjunction with the patient’s PCP to asses and address the behavioral health need.
