Job Description
Responsibilities include Patient Assessment/Care Planning and Counseling, Psychosocial Assessment; KDQOL, Clinical Depression Screen, Referrals to outside agencies, Transportation needs; Health Insurance, Medication coverage, and Collaboration with billing.
Proficiency in written communication is required to complete monthly social work notes, Depression Screening, Quarterly MSPQ’s, and other required documentation.
Social Worker also Participates as a Member of the Interdisciplinary Team to help identify effective interventions and rehabilitation goals to improve patients’ quality of life.

The social worker is responsible for conducting patient assessments, care planning, and psychosocial evaluations. They also collaborate with an interdisciplinary team to develop rehabilitation goals and provide necessary referrals for patient support.
Requirements
The candidate must be a licensed social worker with proficiency in written documentation and clinical screening. Experience in managing patient needs, including insurance and medication coverage, is required.
