Key Responsibilities

Conduct in-home health evaluations for patients, including medication reviews, physical assessments, and diagnostic testing. Build trusted relationships with patients to bridge gaps in care and provide connections to appropriate health services.

Requirements

Must be a licensed physician (MD or DO) with an active, unrestricted license in the coverage area. Ability to obtain and maintain multi-state licenses and travel up to 100% of the time is required.
