Key Responsibilities

• Provide or supervise all social services and participate in the multi-disciplinary team.

• Develop a care plan, if appropriate, with the participation of each participant’s family and other appropriate individuals.

• Provide referral and follow-up for persons not appropriate for ADHC.

• Serve as liaison with the participant’s family and home.

Requirements

• Master’s Degree in Social Work, from an accredited School of Social Work.

• Minimum of 2 years’ experience in the field of gerontology/psychology.

• If ASW, has applied for and received BBS registration and is maintaining registration in good standing including all requirements.

• Strong marketing and outreach skills. Enjoys talking about benefits of the program to community groups and families.
