About the Role

Join Eden Health as a Hospice Nurse Practitioner (NP) in United States.

Key Responsibilities

  • will work autonomously but in collaboration with Physicians to provide primary medical care to patients with chronic or life limiting illnesses that require Palliative or Hospice care interventions.
  • will be responsible for rounding in hospitals and skilled nursing facilities, reviewing patients currently on our services and updating care plans accordingly.
  • will assist with advanced directives and discuss goals of care and the disease process, progression, and grief process with patients and caregivers.
  • will work with the Director of Patient Care Services and Executive Director to provide education on Palliative and Hospice care within the community as well as agency staff regularly.

Requirements

  • Current license from the state Board of Registered Nursing
  • Master’s Degree from an accredited Nurse Practitioner’s Program
  • National Certification as a Nurse Practitioner or certification eligible. Certification must be obtained within the first six (6) months of employment
  • A minimum of one (1) year Nurse Practitioner experience preferred
