Key Responsibilities

This Advanced Practice Pharmacist optimizes complex, specialty, or infusion therapy management within specialty clinics, collaborating with providers to enhance clinical outcomes and operational efficiency. Responsibilities include comprehensive medication management, therapy monitoring, initiating/adjusting therapies based on clinical guidelines, and providing patient education on specialty medications.

Requirements

Candidates must be a graduate of an accredited School of Pharmacy and possess a California Pharmacist license, with an Advanced Practice Pharmacist License strongly preferred. Preferred qualifications include completion of an ASHP accredited residency and previous experience in specialty or infusion therapy management in an ambulatory care setting.
