We are seeking an RN Case Manager to join our Care Coordination team. This community-based nursing role helps patients overcome barriers to care, connect with needed resources, and improve health outcomes. What You’ll Do

* Provide patient outreach, education, and care coordination in clinic, community, and patient home settings.

* Assess barriers to care such as transportation, food access, housing, insurance, medications, and health literacy.

* Connect patients with medical, behavioral health, social service, and community resources.

* Collaborate with providers, care teams, social workers, and community partners.

* Document outreach, referrals, care plans, and patient progress accurately and timely.

What We’re Looking For

* Current nursing license in the state where services are provided; RN preferred.

* Experience in healthcare, care coordination, case management, community outreach, or social services preferred.

* BLS/CPR certification required or ability to obtain within required timeframe.

* Strong communication, organization, documentation, and teamwork skills.

* Valid driver’s license, reliable transportation, and ability to travel locally as needed.

* Bilingual English/Spanish skills preferred but not required.
