Sage Group HealthCare Staffing is seeking RN Hospice for a 13-week travel / contract position in Montrose, CO. Accepting both local and traveler candidates.

Position Details

• Pay Rate: Range $2080 to $2120 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
• Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
• Unit: Home or Community
• Shifts available: Days
• Shift timings: 8 Hours; Days, 8a – 5p

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri with rotating on-call/weekend. Contract offers flexible scheduling

Position Requirements

• Registered Nurse: 1 to 2 years - Required
• Hospice Nursing experience: 1 to 2 years – Required
• EMR/EHR charting - Required
• Must have valid Driver’s License and Personal Auto Insurance

Education, License and Certification Requirements

• ASN or an equivalent – Required
• BSN or an equivalent – Highly preferred
• RN License for State applying or Compact if accepted in the state - Required
• BLS Certification from AHA or American Red Cross - Required

Job Description

Provide Nursing Care for Hospice Patients in private homes, nursing homes, group homes or assisted living facilities.

Manage assigned caseload of patients assigned caseload of patients and lead the interdisciplinary team caring for those patients

Provide assessments, care planning, pain management and symptom management and all other aspects of hospice nursing

Administer Physician-prescribed Medication

• Ensure timely and accurate submission of patient documentation and charting in adherence to client policies.
• Territories to cover: Montrose, CO and surrounding areas
