Hospice Medical Social Worker - Per Diem - Central Coast

Key Responsibilities

The social worker assesses the psychosocial status of hospice patients and families to provide crisis intervention and support. They also facilitate interdisciplinary team meetings and assist with end-of-life planning, including advanced directives and funeral arrangements.

Requirements

Candidates must hold a Master of Social Work (MSW) degree and have at least one year of experience in healthcare or hospice. A valid California driver's license and current CPR certification are required, with a preference for Licensed Clinical Social Worker (LCSW) status.
