Prava Therapy is seeking a full-time Physical Therapist to deliver high-impact, resident-centered care within a premier senior living community in Apex, NC. We are reimagining senior rehabilitation by embedding functional therapy directly into the daily life and culture of the community.

Key Responsibilities

Clinical Excellence: Conduct comprehensive evaluations and deliver goal-driven PT interventions focused on meaningful functional outcomes rather than just exercise minutes.

Community Integration: Serve as an embedded clinical partner, collaborating closely with community leadership, nursing, wellness teams, and families.

Interdisciplinary Leadership: Partner across care teams to identify functional shifts early and support residents across the entire continuum of care.

Caregiver Empowerment: Educate and coach caregivers and community staff on safe mobility techniques, fall prevention, and functional resident support.

Quality & Documentation: Complete precise, timely documentation utilizing modern workflows to reflect true clinical rationale and functional gains.

Licensed PT: Active Physical Therapy license in North Carolina.
