Key Responsibilities

• Maintain a caseload and provide regular case management services to these individuals

• Maintain resident records in compliance with agency and funding standards

• Conduct and review assessments

• Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self

Requirements

• At least 5 years of experience in the mental health field

• Credentialed Alcohol and Substance Abuse Counselor (CASAC) certification is a plus
