About the Role
Join Liquid Personnel as a Social Worker - Older People & Hospital Team in United States. Liquid Personnel is recruiting an Adult Social Worker to join its client’s Older People & Hospital Team, based in Conwy. What will your responsibilities be?
Key Responsibilities
- Lead timely, safe, and person‑centred hospital discharge planning under the Care Act and Discharge to Ass.
- develop effective support plans, prevent delays, and promote independence and wellbeing post‑discharge.
- provide appropriate support to you throughout the process and into your work placement.
Requirements
- qualified experience Degree level or equivalent in Social Work
