Position Summary

The New Jewish Home's Certified Home Health Agency (CHHA) is seeking dedicated and experienced Per Diem Speech-Language Pathologists (SLPs) to join our Community Services team.

In this role, the Speech-Language Pathologist will provide home-based speech therapy services to adult and geriatric patients throughout Manhattan. Responsibilities include conducting comprehensive speech, language, cognitive-communication, and swallowing assessments, developing individualized treatment plans, and providing therapeutic interventions designed to maximize patients' communication abilities, swallowing function, safety, and overall quality of life.

The ideal candidate will be committed to delivering high-quality, patient-centered care while collaborating effectively with an interdisciplinary healthcare team.

Essential Duties and Responsibilities

  • Conduct speech-language and swallowing screenings, evaluations, and assessments as prescribed by the physician.
  • Develop and implement individualized treatment plans to address speech, language, voice, cognitive-communication, and swallowing disorders.
  • Provide therapeutic interventions and rehabilitative services based on each patient's clinical needs and functional goals.
  • Educate patients and family members regarding treatment plans, progress, home exercise programs, and discharge recommendations.
  • Prepare and maintain accurate clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Collaborate with physicians, nurses, therapists, social workers, and other members of the interdisciplinary care team to ensure coordinated, patient-centered care.
  • Participate in case conferences, team meetings, and care planning discussions.
  • Maintain required departmental and regulatory records in a timely and accurate manner.
  • Assist in maintaining therapy equipment, supplies, and work areas.
  • Participate in professional development and educational activities to remain current with best practices and advancements in the field.
  • Perform other duties as assigned.

Key Responsibilities

The Speech-Language Pathologist will conduct comprehensive assessments and develop individualized treatment plans for adult and geriatric patients in their homes. They will provide therapeutic interventions and collaborate with an interdisciplinary team to ensure coordinated, high-quality care.

Requirements

Candidates must be experienced Speech-Language Pathologists capable of performing home-based clinical evaluations and interventions. The role requires strong documentation skills and the ability to effectively communicate with patients, families, and healthcare team members.
