FT Hospice RN Case Manager

Key Responsibilities

The RN Case Manager is responsible for developing and implementing individualized plans of care for hospice patients, including conducting initial assessments and coordinating with physicians. They also provide direct nursing care, educate caregivers, and supervise LPNs and hospice aides to ensure high-quality service delivery.

Requirements

The candidate must be a Registered Nurse (RN) with the ability to perform physical assessments and manage complex care plans. Strong communication skills are required to coordinate with interdisciplinary teams and provide support to patients and their families.
