In This Role
As a Primary Care Physician, you will provide comprehensive, longitudinal patient-centered care in an outpatient setting while collaborating with an experienced multidisciplinary team.
Perform comprehensive patient evaluations, medical histories, and physical examinations
Diagnose and manage a broad range of acute illnesses and chronic medical conditions across a diverse patient population
Develop, implement, and monitor individualized treatment plans with an emphasis on preventive care, chronic disease management, and overall wellness
- Order, interpret and follow up on laboratory studies, diagnostic imaging, and other diagnostic tests
- Prescribe medications and evidence-based treatments consistent with current clinical guidelines
- Provide comprehensive are for adult and geriatric patients (with the opportunity to include pediatric patients based on training and interest)
- Coordinate transitions of care, including hospital follow-up and post-discharge management
- Collaborate with an extensive network of Bayview specialist through a seamless internal referral system
