Key Responsibilities
- Upon admission the social worker will complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
- Facilitate Care Plan meeting with resident/family and interdisciplinary team admission quarterly, annually and significant changes.
- Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long
- term care facility, including their social, emotional, and psychological needs.
Requirements
- Bachelors degree in Social Work, Human Services, or Psychology degree with experience in geriatrics
- Must have one year supervised social work experience (paid/volunteer) in a health care setting working directly with individuals
