Key Responsibilities

• Upon admission the social worker will complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological   needs.

• Upon admission the social worker will determine the resident’s code status, will maintain advance directive paperwork/policies at all times ensuring 100% of facility wide accuracy, and will assist residents/representative in modifying advanced directives when desired.

• Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social worker interventions.

• Facilitate Care Plan meeting with resident/family and interdisciplinary team admission quarterly, annually and significant changes.

Requirements

• Bachelors degree in Social Work, Human Services, or Psychology degree with experience in geriatrics.

• Must have one year supervised social work experience (paid/volunteer) in a health care setting working directly with individuals.

Benefits & Compensation

• Pay rate: $24.00/hr.
