The pharmacist will provide comprehensive medication management to improve patient outcomes in an ambulatory setting, collaborating with a multidisciplinary team. They will specialize in gastroenterology and rheumatology, focusing on disease mitigation, symptom reduction, and the prevention of adverse drug events.
Candidates must have a pharmacy degree from an accredited college and at least two years of recent clinical pharmacy experience in ambulatory care or a one-year residency. A current Minnesota pharmacist license is required, with the ability to obtain a Wisconsin license within six months.
