About the Role
Join Everent Health & Affiliates as a PACE Social Worker in United States. Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age.
Requirements
- Licensed in state of Kentucky as Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW).
- Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
- Two years social work experience in healthcare setting preferred.
- Must maintain CPR certification.
