Job Description

Physical Therapist (PT)

  • Full-Time | Optimal Living Senior Community | Raleigh, North Carolina
  • Prava Therapy

Therapy That Helps People Keep Living Life on Their Terms.

Prava Therapy is seeking a full-time Physical Therapist (PT) to join our team within a premier, luxury resort-style senior living community in Raleigh, North Carolina.

This community emphasizes an "Optimal Living" philosophy—focusing on an active lifestyle, holistic wellness, lifelong learning, resort-style amenities, and continuous engagement. Residents value independence, connection, health, and maintaining an active, fulfilling life.

You'll get to know your residents, their routines, and the activities that matter most to them.

Your Role

As a Physical Therapist, you'll lead clinical physical therapy care, performing evaluations and designing plans of care while embedding yourself as a trusted clinical partner within this vibrant community.

You'll

Perform comprehensive physical therapy evaluations and establish individualized, evidence-based plans of care across adult/geriatric physical therapy domains.

Deliver skilled treatment interventions addressing strength, balance, gait training, transfer mechanics, functional mobility, endurance, joint integrity, and active daily routines.

Target fall prevention, balance recovery, mobility device prescription and training, apartment safety, and environmental navigation.

Introduce, adapt, and train residents, caregivers, and staff on mobility equipment, assistive devices, compensatory movement strategies, and physical safety tools.

Leverage community wellness features—such as fitness centers, indoor pool/hydrotherapy spaces, group activity areas, outdoor pathways, and therapy spaces—to practice real-world functional mobility and engagement.

Identify subtle changes in resident function, safety, or physical status to support proactive care and timely plan-of-care adjustments.

Educate residents, families, caregivers, and community staff on functional independence, fall risk reduction, safe transfers, and mobility support.

Collaborate with interdisciplinary team partners, physicians, community leadership, wellness directors, life enrichment teams, and caregivers.

Monitor resident progress, document evaluations and treatment sessions accurately and in a timely manner, and continuously adjust plans of care based on functional outcomes.

Participate in care coordination, interdisciplinary team discussions, and community wellness initiatives as appropriate.

  • Contribute to a culture of clinical excellence, compassion, professionalism, and continuous learning.
  • WE'RE LOOKING FOR A PT WHO THINKS BEYOND THE TREATMENT SESSION.

You believe great therapy isn't simply about guiding someone through exercise repetitions or completing a routine checklist. It's about helping someone participate fully in the resort-style lifestyle they've chosen.

That might mean walking confidently to wellness classes, dining comfortably with friends, navigating community amenities independently, participating in group activities, or managing daily routines effortlessly.

You Believe

  • Every intervention should have a functional, real-world purpose.
  • Residents should be appropriately challenged—not underestimated.
  • Independence, autonomy, and holistic well-being are clinical priorities.
  • Proactive clinical identification, mobility adaptation, and safety support matter.
  • Assistive device integration, routine structuring, and caregiver education are essential components of skilled care.
  • Functional outcomes matter more than simply completing treatment minutes.
  • Strong clinical relationships lead to better care.

You'll Have a Community — Not Just a Caseload.

Working in a resort-style senior community gives you the opportunity to see residents beyond the treatment session.

You'll see them participating in life enrichment events, utilizing wellness and fitness facilities, enjoying meals in community dining venues, or spending time outdoors. Those real-world interactions provide valuable insight into their functional mobility and physical performance in natural contexts, creating opportunities to support continued independence.

Therapy becomes part of everyday life—not something that exists separately from it.

This Role Is for You If...

You're a Physical Therapist Who

  • Enjoys building genuine relationships with residents.
  • Possesses strong clinical competence in adult/geriatric physical therapy evaluation, treatment planning, movement strategies, and wellness integration.
  • Takes pride in creative, individualized, and functional treatment sessions.
  • Believes therapy should enhance a resident's lifestyle and daily life.
  • Values independence, autonomy, and resident choice.
  • Enjoys collaborating closely with interdisciplinary team members, wellness teams, and community leaders.
  • Is curious, compassionate, and committed to continued professional growth.
  • Communicates clearly and takes ownership of resident treatment outcomes.
  • Wants to make a meaningful impact in an active, upscale senior community.

  • Your treatment plans and sessions are purposeful, engaging, and individualized.
  • And your residents are better able to participate in the activities, amenities, relationships, and routines that make their lives meaningful.
  • That's success.
