Physical Therapist (PT)

Position Summary

The Physical Therapist (PT) evaluates and treats patients referred by a physician to reduce pain, restore function, and improve functional independence. The PT develops individualized plans of care, provides skilled treatment, and supervises Physical Therapist Assistants and Rehabilitation Technicians as appropriate.

Essential Duties & Responsibilities

  • Complete accurate patient evaluations within 48 business hours of referral.
  • Obtain relevant medical history and referring diagnosis.

Develop individualized plans of care with measurable short and long-term goals based on patient needs and desired outcomes.

  • Provide skilled physical therapy using appropriate therapeutic techniques, modalities, and exercises.
  • Re-evaluate patients regularly and modify treatment plans as needed.
  • Monitor patient progress and communicate status and treatment recommendations to physicians.
  • Complete accurate and timely documentation for clinical and billing purposes.

Document patient progress, equipment provided, home programs, education, and communication with physicians and POAs.

  • Maintain patient dignity, privacy, safety, and comfort throughout treatment.
  • Establish positive rapport with patients and clearly explain treatments and their purpose.
  • Follow appropriate precautions and contraindications.
  • Consider the patient's discharge plan and home environment when developing and providing treatment.
  • Coordinate treatment with other disciplines when appropriate.

Determine when maximum functional benefit has been achieved and appropriately discharge patients from skilled therapy.

Communicate professionally with physicians, patients, families/POAs, nursing staff, social workers, occupational and speech therapists, and other members of the interdisciplinary team.

  • Participate in patient-related meetings in accordance with facility policies.
  • Maintain knowledge of applicable insurance and rehabilitation service guidelines.
  • Provide clinical supervision to Physical Therapist Assistants and Rehabilitation Technicians as requested.
  • Maintain a professional appearance and conduct.
  • Contribute to department improvement and assist with implementation of improvements as appropriate.
  • Perform other duties necessary to support the efficient delivery of quality rehabilitation services.
  • Physical Demands
  • Ability to lift up to 35 pounds and safely move, lift, and transfer patients.
  • Ability to stand for extended periods.
  • Adequate fine motor skills for patient care and treatment.
  • Adequate visual acuity to safely interact with patients.

Education & Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the state of practice.
  • Ability to pass criminal background check.
  • Knowledge of applicable physical therapy practices, safety precautions, and insurance requirements.
