Essential Functions
1. Provide comprehensive primary medical care to patients of all ages,
including physical examinations, diagnosing, and treating acute and chronic
illnesses, and prescribing medications.
2. Develop and implement individualized treatment plans based on patient
history, symptoms, and medical records.
3. Collaborate with a multidisciplinary team of healthcare professionals to
ensure coordinated and quality patient care.
4. Educate patients about preventative care, healthy lifestyle choices, and
disease management.
5. Advises patients about long-term treatment and management for chronic
illnesses.
6. Completes all documentation and paperwork in a timely manner.
7. Maintain accurate and up-to date patient records using electronic health
records systems.
8. Stay informed about the latest medical advancements, research, and the best
primary care.

Education and Work Experience
1. Doctoral degree with directly related experience in Family Medicine or
Community Health
2. Board eligible/board certified Family Medicine Physician
3. DEA Certificate.
4. Ohio Licensure or the ability to obtain.
